Job summary

Midland Care Connection Inc. seeks a licensed Occupational Therapist to provide home health services in the Topeka area. You will evaluate patients, design personalized treatment plans, and work with a collaborative care team to promote independence and safe daily living.

Ideal candidates have a Kansas OT license, OT program credential, and experience in home health or related settings. Travel is required within the service area; EMR documentation and OASIS knowledge are a plus.

Qualifications

  • Graduate of an approved Occupational Therapy program and license to practice in Kansas.
  • Experience in home health OT or skilled nursing settings preferred.
  • Strong clinical assessment, documentation, and communication skills are essential.

Midland Care ConnectionMidland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.Home Health ProgramProvides short-term, skilled medical care to individuals in their homes to support recovery, manage chronic conditions, and help patients remain as independent as possible. Services are delivered by a coordinated team of healthcare professionals and may include nursing care, therapy, and education to help patients and families manage health needs safely at home. The program works closely with physicians and caregivers to create personalized care plans that promote healing, prevent hospital readmissions, and improve overall quality of life.Occupational TherapistEvaluates patients in their homes to assess their physical, cognitive, and functional abilities and develops personalized treatment plans to help them safely perform daily activities. They provide therapeutic interventions to improve strength, coordination, balance, fine motor skills, and mobility while teaching patients strategies for completing tasks such as bathing, dressing, grooming, meal preparation, and other essential activities of daily living. The Occupational Therapist also recommends adaptive equipment and home modifications to enhance safety and independence, educates family members and caregivers on proper support techniques, and monitors patient progress by adjusting care plans as needed.
