Job summary

Granite Peak Home Health in Helena, MT is seeking a licensed Occupational Therapist to deliver in-home care to adults and seniors in the Lewis and Clark County area. The role emphasizes patient-centered assessments and personalized treatment plans that support independent living.

You’ll work field-based, schedule-flexible visits with a focus on ADLs/IADLs, safety modifications, and collaboration with an interdisciplinary team, while maintaining accurate EMR documentation and Medicare compliance.

Qualifications

• Montana OT license (MT OT) is required.
• Degree from an accredited Occupational Therapy program (MSOT/OTD).
• Minimum 1 year of home health, skilled nursing, rehab, or acute care experience preferred.
• CPR/BLS certification.
• Valid driver’s license and ability to travel between patient homes.

Responsibilities

• Perform occupational therapy evaluations and OASIS assessments (as applicable).
• Develop and implement individualized treatment plans.
• Provide skilled therapy services in patients’ homes.
• Improve ADLs and IADLs.
• Recommend adaptive equipment, home modifications, and fall prevention strategies.
• Collaborate with RN, PT, SLP, and caregivers.
• Complete EMR documentation in compliance with Medicare and home health regulations.
• Monitor progress and adjust care plans as needed.
