Job summary

Granite Peak Home Health in Polson, Montana seeks a licensed Occupational Therapist to deliver in-home therapy to adults and geriatrics, focusing on improving ADLs and IADLs. This PRN role offers autonomy, flexible scheduling, and meaningful one-to-one patient care in a home health setting.

You will perform evaluations, develop plans, document in EMR per Medicare rules, and travel to patient homes in Polson and surrounding areas.

Qualifications

Active Montana OT license.
Degree from an accredited OT program.
Minimum 1 year experience in home health, skilled nursing, rehab, or acute care (preferred).
Knowledge of Medicare guidelines, OASIS documentation, and home health compliance (preferred).
Strong skills in patient assessment, care planning, and clinical decision-making.
Valid driver’s license and ability to travel between patient homes.
Current CPR/BLS certification.

Responsibilities

Perform occupational therapy evaluations and OASIS assessments (if applicable).
Develop and implement individualized treatment plans.
Provide skilled therapy services in patients’ homes.
Improve patients’ ability to perform ADLs and IADLs.
Recommend adaptive equipment, home modifications, and fall prevention strategies.
Collaborate with an interdisciplinary healthcare team (RN, PT, SLP, caregivers).
Complete accurate EMR documentation in compliance with Medicare and home health regulations.
Monitor patient progress and adjust care plans accordingly.
