Job summary

Granite Peak Home Health in Columbia Falls, MT is seeking a licensed Occupational Therapist to deliver in-home care to adult and geriatric patients recovering from illness, injury, or surgery. This role offers flexible scheduling, autonomy, and meaningful one-on-one therapy in a home health setting.

You will evaluate patients, develop personalized treatment plans, and collaborate with RN, PT, SLP, and caregivers while ensuring Medicare compliance and accurate EMR documentation.

Qualifications

• Active Montana OT license (MT OT License).
• Degree from an accredited Occupational Therapy program.
• Minimum 1 year of experience in home health, skilled nursing, rehab, or acute care (preferred).
• Knowledge of Medicare guidelines, OASIS documentation, and home health compliance (preferred).
• Strong skills in patient assessment, care planning, and clinical decision-making.
• Valid driver’s license and ability to travel between patient homes.
• Current CPR/BLS certification.

Responsibilities

• Perform occupational therapy evaluations and OASIS assessments (if applicable).
• Develop and implement individualized treatment plans.
• Provide skilled therapy services in patients’ homes.
• Improve patients’ ability to perform activities of daily living (ADLs) and instrumental ADLs (IADLs).
• Recommend adaptive equipment, home modifications, and fall prevention strategies.
• Collaborate with an interdisciplinary healthcare team (RN, PT, SLP, caregivers).
• Complete accurate EMR documentation in compliance with Medicare and home health regulations.
• Monitor patient progress and adjust care plans accordingly.
