Job summary

Granite Peak Home Health is seeking a licensed Occupational Therapist to provide in-home OT services for adults and seniors in Butte, MT. The role emphasizes autonomy, one-on-one patient care, and meaningful outcomes in a home health setting.

Qualifications

• Active Montana OT license (MT OT license).
• Degree from an accredited Occupational Therapy program.
• Minimum 1 year experience in home health, skilled nursing, rehab, or acute care (preferred).
• Knowledge of Medicare guidelines, OASIS documentation, and home health compliance (preferred).
• Strong skills in patient assessment, care planning, and clinical decision-making.

Responsibilities

• Perform occupational therapy evaluations and OASIS assessments (if applicable).
• Develop and implement individualized treatment plans.
• Provide skilled therapy services in patients' homes.
• Improve patients' ability to perform activities of daily living (ADLs) and instrumental ADLs (IADLs).
• Recommend adaptive equipment, home modifications, and fall prevention strategies.
• Collaborate with an interdisciplinary healthcare team (RN, PT, SLP, caregivers).
• Complete accurate EMR documentation in compliance with Medicare and home health regulations.
• Monitor patient progress and adjust care plans accordingly.
