Job summary

Granite Peak Home Health is seeking a licensed Occupational Therapist (OT) to provide in-home therapy services in Great Falls, MT. This role focuses on adult and geriatric patient care, delivering customized treatments in patients' homes to improve independence and daily functioning.

The OT will conduct evaluations, develop care plans, and collaborate with a multidisciplinary team while ensuring Medicare-compliant EMR documentation.

Qualifications

• Active Montana OT license (MT OT license) required.
• Degree from an accredited OT program.
• Minimum 1 year experience in home health or related settings preferred.
• Knowledge of Medicare guidelines and OASIS documentation.

Responsibilities

• Perform occupational therapy evaluations and OASIS assessments (if applicable).
• Develop and implement individualized treatment plans.
• Provide skilled therapy services in patients’ homes.
• Improve patients’ ability to perform activities of daily living (ADLs) and instrumental ADLs (IADLs).
• Recommend adaptive equipment, home modifications, and fall prevention strategies.
• Collaborate with an interdisciplinary healthcare team (RN, PT, SLP, caregivers).
• Complete accurate EMR documentation in compliance with Medicare and home health regulations.
• Monitor patient progress and adjust care plans accordingly.
