Job summary
Granite Peak Home Health in Columbia Falls, MT, is seeking a licensed Occupational Therapist to provide in-home therapy to adults and seniors. The role offers flexibility, autonomy, and meaningful one-on-one patient care in a home health setting.
Responsibilities include evaluations, treatment planning, ADL/IADL improvement, and collaboration with a clinical team. Travel between patient homes is required, with CPR/BLS certification kept current.
Qualifications
- Active Montana OT license
- Degree from an accredited OT program
- Minimum 1 year experience in home health or related settings
- Knowledge of Medicare guidelines and OASIS documentation (preferred)
- Strong skills in patient assessment and care planning
- Valid driver’s license and ability to travel between patient homes
- Current CPR/BLS certification
Responsibilities
- Perform OT evaluations and OASIS assessments (as applicable)
- Develop and implement individualized treatment plans
- Provide skilled therapy services in patients’ homes
- Improve ADLs and IADLs
- Recommend adaptive equipment and home modifications
- Collaborate with interdisciplinary healthcare team
- Complete EMR documentation per Medicare and home health regulations
- Monitor patient progress and adjust care plans accordingly
