Job summary

Ignite Rehab (DBA Metro Physical & Aquatic Therapy) is seeking a motivated Occupational Therapist to provide in-home care on a per diem basis in Ocean County, NJ. You will deliver 1:1 therapy, evaluate patients, and develop individualized treatment plans in collaboration with physicians while ensuring compliance and high-quality patient care.

This role offers flexible scheduling, 1099 or W-2 options, and opportunities for professional growth.

Qualifications

• Occupational Therapist Program Graduate.
• Licensed Occupational Therapist in the state of New Jersey or eligible.
• Mandatory Certifications: First Aid, AED, & CPR.
• Home health experience preferred but not required; mentor provided for documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory.
• Strong communication skills with patients, caregivers, and management.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function and prevent dysfunction.
• Perform and document an initial exam, identify problems and determine diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment and adjust as needed.
• Willingness to treat geriatric patients.
