Job summary

MPH Care Services is seeking a licensed Occupational Therapist to provide skilled services in patients' homes in Iowa. The OT collaborates with primary care physicians and RN case managers, performing assessments, developing care plans, and educating families to improve independence and safety.

Ideal candidates hold Iowa licensure, NBCOT eligibility, and at least two years of homecare OT experience, with CPR certification and strong communication with care teams and patients.

Qualifications

• OT degree from ACOTE-accredited program and eligibility to sit NBCOT.
• Current Iowa OT license and CPR certification.
• Minimum of two years OT experience, preferably homecare.
• Criminal background check completed.

Responsibilities

• Receive intake referrals and prepare OT paperwork for visits.
• Perform home safety assessments and OT evaluations.
• Develop and submit Plan of Care to physicians for approval.
• Assess ADLs/iADLs and function with ongoing therapy reviews.
• Supervise Home Health Aides when applicable and ensure compliance.
• Communicate progress with patient, family, and care team.
• Maintain timely, accurate clinical documentation and re-authorization requests.
