Job summary

Freedom in Home Services, based in Omaha, NE, is seeking a Home Health Occupational Therapist to assess clients in their homes, develop individualized care plans, and deliver skilled OT services focused on maintaining independence and quality of life.

You will collaborate with the care team, educate clients and families, monitor progress, and document outcomes while advocating for patient needs and complying with policies and regulations.

Qualifications

• Active OT license in Nebraska
• Minimum 2 years of occupational therapy experience, preferably in home health or a similar setting
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and collaboratively as part of a team
• Compassionate and patient-centered approach to care

Responsibilities

• Conduct initial assessments and develop individualized care plans for clients
• Provide skilled occupational therapy in clients' homes, including interventions and home exercise programs
• Coordinate with care team for integrated care
• Educate clients and families on self-care techniques and health maintenance
• Monitor and document client progress and communicate changes to physicians
• Advocate for clients and ensure needs are met promptly and respectfully
• Adhere to company policies, procedures, and regulatory requirements
