Job summary

MPH Care Services in Iowa seeks a licensed occupational therapist to provide in-home OT services. Work with RN case manager and medical team to assess and treat patients in their homes. You will develop and implement plans of care, evaluate ADLs, and educate patients and families.

Qualifications

• OT education program ACOTE accredited or equivalent.
• NBCOT entry-level exam eligibility/completion.
• Current Iowa OT license.
• Minimum 2 years OT experience, homecare preferred.
• Criminal background check & CPR certification.

Responsibilities

• Receives intake referral information and prepares OT paperwork/tools for visits.
• Performs home safety checks and environmental assessments.
• Develops and submits an effective Plan of Care (POC) to physician.
• Evaluates ADL/iADL abilities and level of function.
• Communicates POC and progression to patient and family.
• Supervises Home Health Aide per state/federal requirements when OT is the primary or sole discipline.
• Documents supervision and therapy progress on a regular basis.
