Job summary

Hackensack Meridian Health’s Visiting Health Services of New Jersey seeks a licensed Occupational Therapist to provide home-based rehabilitation. You will evaluate patients, develop goal-oriented treatment plans, and document care in the EMR while coordinating with physicians and the health team.

The role requires a Bachelor’s in Occupational Therapy and at least one year of OT experience, with a strong emphasis on patient education, discharge planning, and adherence to care plans in a home-care

Qualifications

Bachelor’s Degree in Occupational Therapy and active OT license.
Minimum of one year OT experience.
Proficiency with electronic medical records (EMR) and general computer skills.

Responsibilities

Complete patient evaluation within the specified time frame and ensure a holistic assessment.
Develop an individualized treatment program based on evaluation findings with measurable goals.
Prepare EMR documentation: progress notes, reassessments, discharge summaries with skilled care.
Educate patient and caregiver in home program and provide written instructions.
Contribute to discharge planning and coordinate with health care team.
Complete scheduling, respond to messages promptly, and maintain accurate documentation.
