Job summary

Advantmed seeks a dedicated Nurse Practitioner (NP) in Oregon for full-time work in our provider network. You will perform in-home wellness and risk adjustment assessments across diverse populations, ensuring accurate documentation and high-quality, value-based clinical care statewide.

Travel statewide within a 55‑mile radius of your home location, with two weeks' notice. The role offers a competitive $150,000 salary plus benefits, sign-on bonus, and paid mileage, supporting HIPAA-compliant EMR

Qualifications

• Active NP license in Oregon.
• Minimum 3 years of direct patient care experience, primary care or adult/geriatric preferred.
• EMR proficiency desirable.
• In-home risk assessment experience a plus.

Responsibilities

• Travel to members' homes and conduct comprehensive health evaluations.
• Perform annual wellness visits and health assessments for chronic conditions.
• Document medical history, exams, vascular reviews and medications.
• Coordinate care and generate postvisit summaries for providers.
• Engage in patient education, adherence support and informed decision-making.
