Advantmed is seeking a dedicated Nurse Practitioner to perform in-home wellness and risk adjustment assessments across Oregon, ensuring accurate documentation and high-quality, value-based care for Medicare populations.

This full-time role involves travel within a 55-mile radius of your home and requires an active Oregon NP license with 3+ years of experience, EMR proficiency, and strong patient engagement in preventive health. A generous sign-on and benefits accompany a flexible schedule.

Qualifications

• Oregon NP license active and unrestricted.
• Minimum 3 years of direct patient care experience.
• EMR proficiency preferred; bilingual ability is an advantage.

Responsibilities

• Perform in-home wellness and risk adjustment assessments.
• Travel to members' homes within Oregon and conduct comprehensive evaluations.
• Document findings for post-visit summaries to primary care providers.
• Identify care gaps and support quality and patient experience within the care team.
• Educate patients on preventive health and chronic condition management.
