Job summary

Advantmed is seeking a skilled Nurse Practitioner to join our provider network for in-home wellness risk adjustment assessments of Medicare members and at-risk populations. This 1099 (PRN) role offers flexible scheduling and the chance to impact value-based care.

You will perform comprehensive evaluations, coordinate with care teams, and educate patients in their homes. Travel within Clackamas County, OR, and surrounding area is required, with a minimum of 30 hours per month.

Qualifications

• Must have a valid NP license for the state you'll work in.
• Travel within the assigned county up to 55 miles.
• Prior in-home risk assessment experience preferred.
• Prior 1099 (PRN) experience preferred but not required.
• 3 years patient care experience preferred (primary care/adult/geriatric).
• Must have reliable transportation to travel to member homes.
• Strong EMR experience and ability to work independently.
• Bilingual is a plus.

Responsibilities

• Perform annual in-home wellness visits and risk adjustment assessments.
• Provide individualized quality care to elderly populations in their homes.
• Deliver health education on pain management, medications, etc.
• Build trust with members through empathetic communication.
• Assist in closing quality care gaps (screenings and labs).
• Collaborate to enhance care delivery; commit to a minimum of 30 hours per month.
