Advantmed is seeking a highly skilled Nurse Practitioner to join our provider network for Medicare risk adjustment. This in-home, PRN 1099 role focuses on wellness risk assessments, comprehensive examinations, and documentation to support value-based care.

You will travel to member homes in Jackson County, Oregon (Medford area), about 30 hours per month minimum, with flexible schedule and dedicated coordinator support; travel up to 55 miles from the county.

Competenze

Must have valid NP license for the state you will be working in.
Travel up to 55 miles radius originating in the assigned county.
3+ years patient care experience preferred.
1099, PRN, part-time experience preferred but not required.
Access to reliable transportation for home visits within designated area.
Bilingual is a plus.

Mansioni

Perform annual in-home wellness visits and risk assessment health assessments.
Provide individualized quality care to the elderly population in their homes.
Deliver patient health education on topics such as pain management and medication use.
Build relationships of trust with members through empathy and communication.
Assist in closing quality care gaps (screenings and labs).
Collaborate to enhance care delivery; commit to minimum 30 hours per month.
