Job summary

Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations to members in their own homes, bridging gaps in care and delivering personalized assessments.

You will travel locally within licensed states, perform visits using an iPad, review medications and medical histories, and conduct physical evaluations. Some visits may be virtual; no prescribing is required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Join a national network of clinicians delivering in-home evaluations.
• Travel locally within licensed states, and conduct virtual visits as needed.
• Perform comprehensive health evaluations, review medications and medical history, and conduct physical assessments.
• Provide Diagnostic & Preventive Services as indicated by the health plan.
