Job summary

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. You’ll engage with members in the comfort of their own homes and help bridge gaps in care.

Visits last about 45 minutes and include medication/history review, a physical evaluation, and coordination with health plans for next steps. You’ll travel locally within licensed states and may conduct virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Provide in-home health evaluations using iPad workflow.
• Review medication history and conduct physical evaluations.
• Coordinate with health plans to connect members with the right care.
• Travel locally to member homes as needed.
• Conduct virtual visits when required.
