Job summary

Signify Health is seeking an in-home clinician to perform visits, review medical histories, and conduct physical evaluations for members in their homes. You will operate within a national network dedicated to improving health outcomes, using an iPad and a clinical workflow to deliver care.

Responsibilities include travel within licensed states, performing diagnostic and preventive services, and providing patient education while coordinating with care teams.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities
