Job summary

Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. You will travel across a defined coverage area, with overnight stays possible, and work with a national clinician network to connect patients with the right care.

The role emphasizes patient-centered assessments, no prescription or lab orders by default, and a strong emphasis on safety, quality, and compassionate service.

Qualifications

• Board certification is required.
• Active, unrestricted licenses in coverage areas are required.
• Able to obtain and maintain multi-state licenses as needed.
• Able to cover all counties within the state and flex to other areas as necessary.

Responsibilities

• Bring your heart to every visit, joining a national network of clinicians.
• Visit millions nationwide to connect patients with the right care.
• Collaborate with Signify Health as a clinician colleague.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services as indicated.
