Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations across a defined coverage area, traveling up to 100% of the time. Hold multi-state licenses as required by states and have board certification.

The role includes patient visits at home, reviewing medications and medical history, and arranging diagnostic tests as ordered by health plans. 40 hours per week with comprehensive benefits.

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Able to obtain and maintain multi-state licenses in coverage area, as required
• Able to cover all counties within state and flex to other areas of business need

• Be part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
• Be flexible to travel locally and within licensed states
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
