Job summary

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform In‑Home Health Evaluations. Visits last about 45 minutes with medication/medical history review and physical exams; diagnostic tests may be ordered by the health plan.

An iPad and workflow guide the evaluation; prescriptions or treatment changes are not required. Role requires flexibility to travel locally within licensed states and to conduct virtual visits when needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Engage with patients in their homes for health evaluations.
• Review medications and medical history, perform physical evaluations.
• Order diagnostic tests as indicated by the health plan.
• Use iPad and clinical workflow for each evaluation; no prescriptions or treatment changes required.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Provide Signify Health services including Diagnostic & Preventive Services.
