Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner to provide in-home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
You’ll play a critical role in building trusted relationships to make people healthier, visiting members nationwide with flexible, comprehensive evaluations right at their doorstep. This part-time role includes local travel.
Responsibilities
- Visit health plan members in their homes and perform health evaluations.
- Review medication history and medical history with members.
- Provide diagnostic/Preventive services as indicated and conduct visits as needed.
