Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.
• Per diem, part-time role with flexible hours.

Responsibilities

• Provide in-home health evaluations and assessments.
• Bridge gaps in care by connecting members to the right services.
• Travel locally within licensed states as needed; may conduct virtual visits.
• Use iPad and clinical workflow to document evaluations.
