Job summary

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations in members' homes, bridging gaps in care and guiding them to the right care.

The role supports a national network of clinicians; requires travels within licensed states and local travel, with part-time hours and a base pay range for the role. The visit lasts about 45 minutes and uses an iPad for a streamlined workflow.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
• Able to cover the following primary locations: Sterling and surrounding area.

Responsibilities

• Provide in-home health evaluations for plan members in their homes.
• Bridge gaps in care by connecting members to the right care.
• Use iPad and clinical workflow; no prescribing or lab orders required.
