Job summary

Optum Home & Community Care in Texas is seeking a part-time Nurse Practitioner to conduct in-home health assessments for Medicare Advantage and other plan members. You will identify diagnoses, educate members, and collaborate with primary care providers to close gaps in care.

This field-based role emphasizes safe home visits, patient education, and coordination of care with PCPs, with a flexible 24-hour per week schedule and no on-call obligations.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted Texas license (or ability to obtain by start date) without disciplinary actions.
• Access to reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home assessments including medical history, vitals and exam.
• Educate members on disease processes, medications and adherence.
• Document diagnoses for care management and communicate findings to PCPs.
• Identify urgent situations and intervene appropriately.
• Address social determinants of health and provide referrals as needed.
