Job summary

Flexible part-time schedule (20 HPW) with no on-call, weekends, or holidays. Requires state licensure and reliable transportation for home visits.

Qualifications

• Active NP or PA license and national certification as applicable.
• Experience in home health or geriatric care preferred.
• Ability to work independently in patients' homes, with reliable transportation.

Responsibilities

• Perform in-home health assessments for Medicare Advantage and other plans.
• Review medical history, medications, vital signs, and exams during visits.
• Document diagnoses and coordinate with PCPs to close care gaps.
