Job summary

Advantmed is seeking a dedicated Nurse Practitioner (NP) to join our full-time provider network in Oregon. In this role, you will perform in-home wellness and risk adjustment assessments to support comprehensive, value-based clinical care across the state.

You will travel to members’ homes, conduct wellness visits, and document findings to support high-quality care. A current NP license in Oregon and at least 3 years of direct patient care are preferred; EMR proficiency is desirable, with

Responsibilities

• Travel to members’ homes and conduct comprehensive health evaluations.
• Conduct Annual Wellness Visits and health assessments for members with chronic conditions.
• Perform medical history, physical exams, review of systems, vital signs, and medication reconciliation; include targeted screenings (HbA1c, spirometry, KED, DRE, FIT, bone density).
• Educate patients on preventive health and adherence; support informed decision-making and goals of care discussions when appropriate.
• Identify diagnoses for care management and risk adjustment; communicate post-visit summaries to PCP; close care gaps.
• Travel statewide across Oregon; travel within a 55-mile radius of home location;州wide travel with notice.

Primary Purpose

Your work will ensureaccurate, comprehensive documentation and support high-quality,value-basedclinical care across the state.

Key Responsibilities

In-homeClinical Assessments

• Travel to members’ homes and conduct comprehensive health evaluations

• Conduct Annual Wellness Visits and health assessments for members with chronic conditions.

• Perform:

• Medical history and physical examinations

• Review of systems

• Vital signs collection

• Medication review and reconciliation

• Possible preventive health screenings

• Hemoglobin A1C

• Spirometry

• KED (Kidney Health Evaluation)

• DRE (Diabetic Retinal Exam)

• FIT

• Bone Density

Patient Education & Engagement

• Deliver preventive health education and counseling opportunities.

• Educate patients on ongoing health monitoring, medication adherence, and chronic condition management.

• Support informed decision-making and ensure true, informed consent.

• Engage in goalsofcare discussions whenappropriate.

Care Coordination & Quality Support

• Identifyclinically relevant diagnoses for care management and risk adjustment documentation.

• Communicate findings used to generate postvisit summaries for the primary care provider.

• Identifyand help close quality care gaps (e.g., screenings, labs, follow-ups).

• Collaborate enthusiastically with the broader care team to enhance clinical quality and patient experience.

Travel Requirements

• Full-time providers support the entire state of Oregon.

• Daily travel expectationsremainwithin a 55-mile radius of the assigned home location.

• Statewide travel will be scheduled with a minimum of two weeks’ notice.

Compliance & Professional Standards

• Adhere to HIPAA regulations and safeguard protected health information (PHI).

• Maintainaccurate,timely, and compliant EMR documentation.

• Performadditionalduties as assigned in alignment with organizational goals.

Qualifications

• Active, unencumbered Nurse Practitioner (NP) license in Oregon.

• Minimum 3 years of direct patient care experience (primary care, adult/geriatric preferred).

• EMRproficiencyis desired.

• Priorin-homerisk assessment experience is a plus but notrequired.

• Bilingualproficiencyis an advantage.
