Qualifications

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted licensure in the practicing state
• Active and unrestricted driver's license with reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire
• Ability to gain a collaborative practice agreement where required
• Ability to navigate varied environments and patient home settings

Responsibilities

• Conduct comprehensive in-home assessments including history, meds, vitals, and exams
• Manage patients during acute illnesses or chronic condition exacerbations
• Oversee short-term sub-acute rehab episodes
• Ensure accurate documentation including ICD-10
• Collaborate with interdisciplinary care team and provide counseling/education

Skills
