Qualifications

• Active NP or PA license in good standing.
• ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care, or NCCPA for PAs.
• Ability to lift up to 30 pounds and perform physical assessments.
• Reliable transportation for home visits.

Responsibilities

• Conduct in-home assessments including medical history, vitals, medication reconciliation and physical exams.
• Perform point-of-care testing and screenings as appropriate.
• Identify diagnoses for care management and communicate findings to PCPs.
• Educate members on disease processes, medications, and compliance.
• Travel up to 85% for home visits and coordinate care across settings.
