Job summary

UnitedHealth Group's Optum Home & Community Care unit is building an integrated care model to address patients' physical, mental, and social needs with in-home assessments and a flexible schedule. Travel up to 85% is required, with no on-call, weekends, or holidays.

The role requires active NP or PA licensure, national certifications (Family/Geriatrics/Adult or related), and the ability to perform physical assessments and transport a 30-pound bag.

Qualifications

• Active NP or PA license or ability to obtain by start date.
• Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or NCCPA for PA.
• Ability to lift a 30-pound bag, navigate stairs and perform physical assessments.
• Reliable transportation to complete home visit assessments.

Responsibilities

• Conduct in-home assessments including medical history, review of systems, medications, vitals, and physical exam.
• Perform evidence-based screenings including point-of-care testing when appropriate.
• Identify diagnoses for care and active medical management to guide treatment.
• Communicate assessment findings to inform the member's PCP and close care gaps.
• Identify urgent and emergent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence to treatment.
• Travel up to 85%.
