Job summary

Matrix Medical Network invites licensed nurse practitioners to join our nationwide in-home health assessment team. You will perform comprehensive health evaluations in patient homes or SNFs, with appointments scheduled to fit your availability.

Qualifications

Master’s degree in nursing and NP licensure in the practicing state.
Current NP licensure and prescriptive authority or ability to obtain.
Board certification in Adult/Adult Gerontology/Family nursing per listed boards.
BLS, ACLS or CPR certification currently valid.
1 year NP experience preferred; new grads welcome.
Valid state driver’s license and auto insurance coverage as required by state.
Strong computer skills and familiarity with employee health/medical record software
Excellent verbal and written communication skills

Responsibilities

Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education and referrals.
Administer and order point-of-care and lab-based testing as needed.
Collaborate with physicians, case managers, social workers, family members and other healthcare staff.
Coordinate with PCP on patient education and follow-up as necessary.
Provide services at the member’s home or Skilled Nursing Facilities
