Qualifications

• Master’s degree in nursing is required.
• Current NP licensure and board certification.
• BLS/ACLS certifications.
• At least 1 year NP experience.
• Ability to travel extensively and a valid driver’s license.
• Strong communication and interpersonal skills.

Responsibilities

• Perform comprehensive in-home health assessments reviewing medical history, medications, social factors and risks during visits.
• Administer tests and document findings thoroughly.
• Collaborate with healthcare teams and provide patient education.
• Conduct visits at patients’ homes or facilities; visits can last up to one hour.
