Job summary

A healthcare solutions provider is seeking a highly skilled and compassionate certified Nurse Practitioner to conduct in-home wellness risk adjustment assessments. The role offers great flexibility with a competitive compensation of around $100 per completed assessment and provides support for appointment confirmations and member scheduling. NP candidates must have a valid license and previous in-home assessment experience is preferred. This is a PRN role with a daily travel requirement within a 55-mile radius.

Qualifications

• Must have a valid unencumbered NP License for the state you will be working in.
• Previous in-home risk assessment experience preferred.
• 3 years patient care experience preferred (primary care/adult/geriatric, EMR).

Responsibilities

• Conduct in-home wellness risk adjustment assessments for Medicare members.
• Perform medical history and physical examinations.
• Deliver preventive health education and counseling.
