Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform comprehensive in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve health outcomes.

The position offers flexible scheduling and a $10,000 sign-on bonus, with no on-call, weekends, or holidays required. Must hold active NP/PA certification and state licensure to practice in the assigned state.

Qualifications

• ANCC/AANP NP certification or NCCPA PA certification and active state licensure
• Proficiency with electronic medical records systems
• Experience in home health or Medicare population desired

Responsibilities

• Conduct in-home comprehensive health assessments.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to close gaps in care.
• Identify urgent situations and intervene appropriately.
• Educate patients on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
