Job summary

Matrix Medical Network recruits Nurse Practitioners to perform comprehensive in-home health assessments across the nation. Roles involve reviewing medical history, medications and social factors to close care gaps within a single visit. You will order tests, educate patients and coordinate with PCPs and care teams.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult, Gerontology or Family NP specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance for state of residence.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnosis, treatment, health education, referrals and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other care team members.
• Provide services at the member’s residence, such as home or Skilled Nursing Facilities.
