Job summary

Optum Home & Community Care in Louisiana is seeking an Advanced Practice Clinician (NP/PA) to conduct annual in-home health assessments for Medicare Advantage and other plan members. This non-prescribing, field-based role focuses on education, gap closure, and collaboration with primary care providers.

You will perform comprehensive in-home assessments, document diagnoses, educate members on disease processes and medications, address social determinants of health, and provide referrals as

Qualifications

Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
Active, unrestricted licensure in Louisiana with no disciplinary actions.
Active driver's license and reliable transportation for home visits.

Responsibilities

Conduct comprehensive in-home assessments including vitals, meds reconciliation, and history.
Identify and document diagnoses for care management and treatment planning.
Educate members on disease processes, medications and adherence.
Coordinate with PCPs to close gaps in care and address urgent situations.
Address social determinants of health and provide referrals as needed.
