Job summary

Hueman is seeking Nurse Practitioners to conduct in-home Health Risk Assessments for Medicare and Medicaid members around Greenville, OH. This role offers a flexible schedule and the freedom to work on your terms while traveling to patients’ homes.

Qualified NPs will perform non-invasive assessments, review histories, and provide health education. Licensure, board certification, and BLS are required; home-based care does not involve prescribing in the home.

Qualifications

• Masters degree or commensurate NP experience and licensure
• Active State NP license
• Active National board certification
• Current BLS certification
• 12 months of experience as a paid advanced clinical provider preferred but not required

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventive healthcare measures.
