Job summary

Hueman is seeking qualified Nurse Practitioners to conduct in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Niles, OH. This flexible role offers travel between patient homes, non-invasive assessments, documentation, and patient education.

NPs are not required to treat or prescribe in the home, and training and scheduling support are provided. Candidates should hold an active NP license, board certification, BLS, and a master’s degree; 12 months of paid

Qualifications

• Masters Degree in Nursing (NP) and active NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider preferred.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventative care.
