Hueman is seeking Nurse Practitioners to perform in-home Health Risk Assessments for Medicare/Medicaid members around Columbus, GA. This role offers a flexible schedule and travel between patient homes, with comprehensive training and scheduling support to help you balance professional and personal goals.

Qualified NPs will complete non-invasive assessments in patients' homes, maintaining thorough documentation and ensuring safety through medical reconciliation.

Qualifications

• Masters degree in nursing with NP credential.
• Active state NP license.
• National board certification as NP.
• BLS certification is required.

Responsibilities

• Travel to and from patients' homes to perform HRAs.
• Conduct comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate, timely documentation.
• Perform medical reconciliation to ensure patient safety and proper care.
• Provide basic patient education on wellness and preventive care.
