Job summary

Hueman is actively hiring Nurse Practitioners to conduct in-home Health Risk Assessments for Medicare and Medicaid members in and around Jeromesville, OH. This role offers travel, a flexible schedule, and the autonomy to work outside a clinic setting.

Nurse Practitioners will perform comprehensive, non-invasive assessments, review medical histories, and document with accuracy. Travel reimbursement helps balance care and home visits, with training provided.

Qualifications

• Master's degree in Nursing or commensurate experience.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider preferred but not required.

Responsibilities

• Travel to and assess patients in their homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document accurately.
• Conduct medical reconciliation to ensure safety.
• Provide basic wellness education and preventive guidance.
