Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members in and around Akron, OH. This flexible role lets NPs schedule visits around their preferences while supporting community healthcare needs.

Qualified candidates will have an NP license, board certification, and a current BLS certificate, with 12 months of paid advanced clinical provider experience preferred. Travel and training are provided.

Qualifications

• Master's Degree in Nursing or commensurate experience with NP license.
• Active State NP license.
• Active National board certification.
• Current BLS certification.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventative care.
