Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members around Mansfield, OH. This flexible role blends clinic knowledge with community outreach, allowing you to prioritize patient connection while contributing to preventative care.

Qualified NPs with an active license and board certification will travel to patients' homes, conduct non-invasive assessments, review histories, and document findings for quality care.

Qualifications

• Masters Degree or commensurate experience with NP license.
• Active State NP license is required.
• Active National board certification is required.
• Current BLS certification is required.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative care.
