Job summary

UnitedHealth Group's Optum Home & Community Care is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform annual in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

You'll work in a flexible schedule with 8am–8pm coverage across a 7-day week, with no on-call, no weekends and no holidays.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in Texas (or ability to obtain by start date).
• Access to reliable transportation to complete home visits.

Responsibilities

• Conduct in-home assessments including history, vitals, exam and screenings.
• Identify diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close gaps in care.
• Recognize urgent situations and intervene.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals.
