Job summary

KeystoneCare Home Health and Hospice is seeking a compassionate RN or LPN Caregiver to provide skilled in-home nursing for a patient with complex medical needs (tracheostomy, ventilator, PEG tube) in the Ambler/Blue Bell, PA area. Work closely with the care team to deliver high-quality, patient-centered care in the home.

Qualifications

• Graduate of an accredited Licensed Nursing program.
• Current Pennsylvania Nursing license in good standing.
• Ability to read, write, and communicate effectively in English.
• Strong organizational and time-management skills.
• Experience with home health care and complex medical needs preferred.

Responsibilities

• Provide skilled nursing care for a patient with a tracheostomy, ventilator, and PEG tube.
• Monitor and assess the patient’s physical, emotional, and clinical condition.
• Execute the patient care plan and communicate with the RN Case Manager.
• Administer medications and treatments as prescribed.
• Assist with tracheostomy care, ventilator monitoring, and enteral feeding support.
• Provide personal care and activities of daily living assistance.
• Assist with mobility, transfers, and use of medical equipment.
• Prepare meals and assist with eating and hydration as appropriate.
• Perform light housekeeping and maintain a safe home environment.
• Accompany the patient to medical appointments and community outings.
