Job summary

Prolific Homecare LLC in Bucks County, PA is seeking an RN or LPN to provide one-on-one nursing care for a Developmentally Disabled (ODP) client in Bucks County. This is a home-based, in-person position with full-time or part-time options, seven nights per week.

Responsibilities include tracheostomy care, G-J tube care for medications and hydration, daily living activities, and documenting care while coordinating with the interdisciplinary team.

Qualifications

• Active, unrestricted PA RN or LPN license.
• Experience with tracheostomy care and G-J tube management preferred.
• Valid driver’s license and reliable transportation required.
• Compassionate, patient-focused home-care professional.
• Ability to follow care plans and maintain thorough documentation.

Responsibilities

• Provide skilled nursing care per physician orders and care plan.
• Ensure health, safety, dignity, and well-being of the client.
• Observe, report, and document changes in client’s condition.
• Maintain compliance with ODP regulations and agency policies.
• Collaborate with family and healthcare professionals to support quality care outcomes.
