Job summary

KeystoneCare is seeking an experienced Licensed Nurse to provide in-home care for a patient with a tracheostomy, ventilator, and PEG tube in the Ambler/Blue Bell, PA area. The role emphasizes one-on-one patient care, collaboration with the care team, and safety in the home environment.

Qualifications

• Graduate of an accredited Licensed Nursing program.
• Current Pennsylvania Nursing license in good standing.
• One year of home health experience preferred.
• Ability to read, write, and communicate effectively in English.

Responsibilities

• Provide skilled nursing care for a patient with a tracheostomy, ventilator, and PEG tube.
• Monitor and assess the patient’s physical, emotional, and clinical condition.
• Administer medications and treatments as prescribed and authorized.
• Assist with personal care, including bathing, grooming, dressing, toileting and hygiene.
• Follow care plan and communicate changes to the RN Case Manager.
