Job summary

Advantmed, LLC is looking for a Nurse Practitioner to conduct in home assessments for Medicare members. This is a PRN position offering ~100 per assessment, with flexible weekdays and no weekends or on call.

You will perform comprehensive assessments and screenings to educate members on their health. A valid NP license is required, alongside a preference for candidates with patient care experience.

Qualifications

• Active, unencumbered NP license in the applicable state.
• 3+ years of patient care experience preferred.
• Comfortable with EMR systems and independent work.
• In home or risk adjustment experience preferred.

Responsibilities

• Conduct in home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, medication reviews, and screenings.
