Job summary

Advantmed, LLC is seeking a Nurse Practitioner to conduct in-home wellness assessments for Medicare members in Seattle, Washington. Qualifications

• 3+ years of patient care experience preferred.

Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, medication reviews, and screenings.
• Educate members on preventive care and chronic disease management.
