Job summary

Advantmed, LLC is seeking a licensed Nurse Practitioner to conduct in home assessments for Medicare members. This PRN role offers flexible scheduling, competitive pay of approximately $100 per completed visit plus mileage. Ideal candidates will have an active NP license, at least 3 years of patient care experience, and must be comfortable working independently. The position requires performing assessments, educating members on healthcare, and documenting in EMR. Travel within a 55-mile radius of the assigned county is expected.

Qualifications

• Active, unencumbered NP license in Missouri.
• 3+ years of patient care experience preferred.
• Comfortable with EMR systems and independent work.
• In home or risk adjustment experience preferred.

Responsibilities

• Conduct in home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, medication reviews, and screenings.
• Educate members on preventive care and chronic disease management.
